You are allowed to submit this form completely anonymously. Please note that without contact information, the Commission may find it more difficult to conduct a thorough investigation.

To assist the Commission in thoroughly evaluating your complaint, please include as much detailed information as possible. Specifically, the Commission requests that you identify, to the best of your ability, the alleged violator, describe the alleged violation, and specify the date or time period during which the violation is believed to have occurred.
