The Village is offering a waitlist for seniors who would like to be contacted in the event that a dose becomes available because another participant is unable to attend. Please complete the form below to be added to the waitlist. Clinic staff will contact individuals on the waitlist only if a vaccine dose becomes available. The waitlist is only for this River Forest clinic and is not a waitlist for doses elsewhere. If a call goes unanswered, the next individual on the list will be contacted. Please provide a phone number where you can be reached on February 26 between the hours of 9 a.m. and 4 p.m.

Dear River Forest Residents,

Registration for the COVID-19 vaccine clinic in River Forest has reached capacity and is now closed. Appointments were available on a first-come, first-served basis and were limited to the number of available doses. 

The Village is offering a waitlist for seniors who would like to be contacted in the event that a dose becomes available because another participant is unable to attend. Please fill out the waitlist form below. Clinic staff will contact individuals on the waitlist only on February 26 if a vaccine dose becomes available. The waitlist is only for this River Forest clinic and is not a waitlist for doses elsewhere. If a call goes unanswered, the next individual on the list will be contacted. COVID-19 Vaccination Clinic for Ages 65 and Older

A Special E-News Message from the Village President, Catherine M. Adduci

Dear River Forest Residents,

The Village of River Forest has partnered with Jewel Osco and School District 90 to provide a free COVID-19 Vaccine Clinic utilizing the Pfizer vaccine for individuals ages 65 and older.

  • When: February 26 (first dose) and March 19 (second dose). Appointments will be offered between 9 a.m. and 4 p.m. 
  • Who: Individuals age 65 and older. Please register only if you know you are available on February 26 and March 19 to receive both vaccine doses. Limited vaccines are available and sign-up will be offered while supplies last. 
  • Where: Participants will report to the First Presbyterian Church parking lot for a drive through check-in 15 minutes before their appointment time and will then be directed to Roosevelt Middle School to receive the vaccination. First Presbyterian Church is located at 7551 Quick Avenue.
  • How: If you are 65 or older and are interested in this opportunity, please click here to register. River Forest residents who need help registering or are unable to make arrangements for their own transportation can contact the River Forest Senior Response Team at 708-613-1642.

Appointments are available on a first-come, first-served basis and are limited based on the number of doses available. 

All individuals in Illinois age 65 and older can receive the vaccine regardless of insurance coverage or immigration status, including individuals who are undocumented.

Instructions

ALL PARTICIPANTS MUST REPORT TO THE FIRST PRESBYTERIAN CHURCH PARKING LOT.

Please only report to the First Presbyterian Church parking lot 15 minutes before your appointment time. Only persons with scheduled appointments will be admitted. 

Please arrive from Lathrop Avenue (500 Block) and turn into the First Presbyterian parking lot. Event staff will direct you from there. 

If you have a disabled person parking placard, please bring that with you. 

Participants will then be directed where to park near Roosevelt Middle School where they will be checked in and receive the vaccination. Vaccinations will be administered in the gymnasium.  Participants will be required to stay in designated areas for a 15-20 minute observation period before being allowed to leave. 

All participants must wear a mask and maintain social distancing whenever possible. 

Participants should: 

  • Wear short sleeve shirts/clothing that will allow for easy administration of the vaccine
  • Bring a photo identification with birthdate. 
  • Bring a completed consent form. Participants who bring a completed copy will help to aid in the flow of the event. Anyone who is unable to print and complete the form ahead of time will be provided with a copy upon arrival. 
  • Bring an insurance card (including Medicare or Medicaid). This event is free.

Participants will receive the second dose on March 19 at the same time as their first appointment. Participants do not need to schedule a second appointment for the second dose.
